The Tenth District Achievement Week Committee is accepting nominations for the various awards outlined in Form 77 for the period 11/1/2010 – 10/31/2011. You can download both Form 77 and 76 on the 10th District Website (http://www.10thdomegas.org/?page_id=37). We are also accepting essays for the National High School Essay Contest and Service Recognition Awards. Submit Form 76 for Service Awards. Carefully read the general rules for completing Form 77. Please NOTE that all events and/or activities listed must be dated reflecting the year of the event or activity. Events or activities that are not dated will not be considered in the scoring. For Tenth District Lifetime Achievement nominations, use Form 77, and check the “Founder’s Category.”
Lifetime Achievement Criteria
- Be a brother from the Chapter; does not have to be financial but must not be expelled, suspended, or on probation
- Have a combination of a minimum of 25 years in the fraternity and 50 years of age
- Has shown sustained contributions and achievements over several years to reflect lifelong service to the fraternity and
- Should be recognized on the sum total of his efforts and activities in and outside of the fraternity, along with exhibiting a willingness to serve his fraternity, community and mankind
The deadline for electronic submission is midnight, Monday, March 5, 2012. There will be no exceptions to this deadline. Submit all material to Achievement Week Chairman Bro. Charles H.
